step 1:  cut a hole in the box

step 2:  look for an existing thread in which to nominate said post/image/rant/whatever - if none found, create a new thread with the name of the award (e.x. " *Award* Best Thread Title of the Year "), and publish your nomination (e.x. "How to nominate: a w00kieguide").  all subsequent nominations for that award shall be posted in that thread.

step 3:  make her open the box


and that's how ya do it, baby

ok, it is looking like we may have more nominations than usable for some categories... how about a limit of 5?  if we have more than 5 nominated, a special panel of 3 volunteers shall narrow it down to 5.  if that doesn't work for everyone, then we will need to have run-off polls to determine the top 5.  again, all polling should be secret (you can set the poll options when created), with only the top 5 vote getters eligible for nomination.
